Salary: £27,572 per annum
Hours: 37 hours per week

We are looking to recruit a Coordinator with the ability to develop effective team working with a passion for providing a range of support to Sandwell residents aged 18 plus including support to live independently upon discharge from hospital.
The Coordinator is responsible for triaging referrals, conducting initial assessments, liaising with multidisciplinary professionals for data gathering, and coordinating appropriate interventions such as transport, supporting recovery and crisis support.

Knowledge, Skills and Qualifications required:
Health & Social Care Diploma/NVQ Level 5 or equivalent qualification with experience in a health and social care service environment.
Proven experience of the ability to work collaboratively with partnership organisations
Managing time effectively and responding appropriately to a range of competing demands.
Assessing client needs at pace.
Working within multi-disciplinary teams.
Relevant community-based experience.
