Job Title: Dementia Navigator
Salary: £22,874 per annum (FTE £24,182 per annum)
Location: The Bridge, St Marks Road, Tipton
Hours: 35 hours per week
Contract: Fixed term contract to 28th June 2023
The Sandwell Community Dementia Support Service (SCDSS) is a new service commissioned by Sandwell Metropolitan Council and the Black Country and West Birmingham Clinical Commissioning Group serving Sandwell Metropolitan Borough.
As the lead provider, we are working alongside eight other local organisations; Agewell CIC, Dementia Pathfinders CIC, West Bromwich African Caribbean Resource Centre, Murray Hall Community Trust, St Albans Community Association, BUDS, Age UK Sandwell and Alzheimer’s Society to provide this service across Sandwell Metropolitan Borough.
We are establishing a new team to work with existing town-based Dementia Advisors. Working closely with GPs and primary care teams, the Dementia Navigator will ‘case-find’ people, living in local communities and nursing and residential care homes, experiencing memory problems and other signs and symptoms of dementia, but with no formal diagnosis.
These will be key roles in carrying out initial consultations most suitable for the person (in person, virtually, by phone) to assess needs, facilitating screening of people with memory concerns in a timely manner, in partnership with primary care, using the most appropriate screening tool for the setting the person is living in and co-developing individualised support plans with people living with dementia, harnessing support from families and partner organisations.
A dementia care Level 2 qualification or equivalent is essential and successful post holders are required to achieve Level 3 whilst in post.
