The Community Offer Plus service provides support to adults over the age of 18 across Sandwell. The service includes information, advice and guidance to help you manage your needs.

 

Our mission is to bring together a network of organisations to support Sandwell residents through a variety of free, essential services, made possible through funding from Sandwell Metropolitan Borough Council and the NHS.

We work hard to find people in our community who might need extra help and support. Our team regularly visit local community centres and talk with doctors, nurses, social workers, and community groups to identify those who would benefit.

We explain available services in simple terms and can go with you to important appointments if you'd like. We help with filling out forms and make sure all your health and care providers are talking to each other to provide the best possible outcome for you. For instance, if you need help getting to medical appointments, we can arrange transport and even accompany you if you would find that helpful.

We create a personal plan just for you, based on what you need to stay independent and reduce isolation. This includes matching you with friendly volunteers who will support and guide you to help you find local groups and activities you might enjoy and also access to local services.

Financial challenges can be overwhelming, and we understand. Our team doesn't just offer advice—we provide a compassionate assessment of financial needs and connect people with local welfare support and benefits guidance. We're here to help people not just survive their current crisis, but develop strategies to prevent future financial stress.

Sometimes we all need someone to talk to, and our team are here to provide that listening ear. We offer regular check-ins to see how you're doing and provide support through difficult times. We understand that loneliness can be challenging, so we focus on helping you build connections in your community. This might mean arranging for a volunteer to call you for a friendly chat each week or helping you join a local coffee morning where you can meet new people.

Crisis Support and Supporting Recovery

Picture a proactive approach where we're constantly connecting the dots across healthcare systems. We're not just waiting for referrals—we're actively seeking out individuals who need support. Our team works seamlessly with Acute Wards, the Integrated Discharge Hub, SMBC Enquiry, Care Navigation Centre, and Harvest View, using smart, pro-active case finding techniques to identify those who might slip through the cracks.

We understand that getting home after a hospital stay isn't just about the destination—it's about the journey. Our transport service ensures smooth transitions, helping individuals get from hospital or community beds back to their place of residence. We'll collect and return small equipment that fits in a standard car, and we can even provide transport to appointments or community activities as outlined in their support plan.

Our support doesn't end when you walk through your front door. We offer up to 4 weeks of dedicated care, working hand-in-hand with health and social care community services. Our goal? To support your reablement and rehabilitation, keeping you safe at home and reducing the risk of hospital or care home readmission.

Medication can be complicated, so we're here to help. We'll collect prescriptions from the pharmacy or community health services and even support long-term prescription delivery arrangements, ensuring you never miss a crucial medication.

Transitioning from intermediate care to self-care isn't always easy. That's why we collaborate with iCares to provide tailored exercise support, helping you regain independence and confidence.

We know daily tasks can be challenging during recovery. We offer time-limited support for food shopping, helping you either shop in person or order online, ensuring you have what you need.

Service Exclusions

To ensure we provide the most appropriate support, we have some specific service boundaries:

  • Individuals under 18 (with exceptions for those transitioning to adult services at 17)

  • Cases with complex behavioral risks

  • Hands-on personal care

  • Respite care

  • Sitting services

  • Cooking and cleaning duties (though we can signpost)

  • Situations with unsuitable or unsafe living environments

  • Heavy manual handling

  • Handling bank cards or withdrawing cash without the service user present

Operating Hours

Living Well Core Services:

Monday to Friday: 9 am to 6 pm
Supporting Recovery Core Services:

7 Days a Week: 9 am to 8 pm
(last referral at 6:30 pm)
Crisis Support Core Services / Admission Avoidance:

7 Days a Week: 9 am to 8 pm (last referral at 6:30 pm)


Additionally, an out-of-hours response is available via email: bcicb.communityoffer@nhs.net and
voicemail at 0121 612 2934 to direct callers to appropriate emergency services.
